Community Connect Transport provides safe and reliable transport services to eligible clients, who live independently within the Lower North Shore and are unable to access mainstream transport. The service covers the NSW Local Government Areas of North Sydney, Lane Cove, Mosman and Willoughby.
Community Connect Transport offers a “point to point” service for local residents. Our service operates Monday to Friday from 8am to 4pm and provides transport to and from your destination. You can book as your circumstances require, it may be to get to an appointment (medical, non-medical or personal), visit the shops, the hairdresser, the local Community Centre or attend a social get together with family and friends.
Our clients include frail older people, people with disabilities, people from culturally and linguistically diverse backgrounds, children, youth, seniors, indigenous Australians and Carers. All our vehicles are selected for ease of access and many are wheelchair accessible, enabling passengers with different levels of mobility a choice of transport.
Community Connect Transport services are funded for either older people who need assistance with daily living to keep living independently at home, or to younger people who have a permanent functional disability. For people over 65 years of age and for Aboriginal and Torres Strait Islanders over 50 years of age, the Australian Government provides transport help through MyAgedCare. To check your eligibility for our services either simply call the MyAgedCare contact centre on 1800 200 422 or visit their website: www.myagedcare.gov.au As a registered NDIS Service provider Community Connect Transport can assist NDIS clients with transport options. Our fully accredited drivers are trained to assist with your needs.
Medical Appointments:
We provide transport to and from medical, hospital, specialist and rehab appointments in either cars or buses depending on your needs. Please note we do not provide hospital in-patient discharge transport.
Shopping:
Our weekly Shopping Services provide clients with reliable, regular access to the shops. You can book a permanent pick up, or travel occasionally. Carers can accompany you at no extra cost. A limit of two shopping bags per person applies. Social Occasions We organise a variety of social outings and events every week which provide great opportunities to get out and about, meet new people and build friendships.
