Our team supports you by creating your care plan, setting your care goals, and managing the financial aspects of your package, including invoice payments, reimbursements, and compliance checks for your workers. Self-management is ideal for those who want to feel empowered in managing their Home Care Package.
Our services
Trilogy Care is a registered Home Care Package provider, offering two levels of self-managed home care: Self Managed and Self Managed PLUS.
Self Managed
The Self Managed option gives you full control over your Home Care Package. This service includes:
- A dedicated Care Partner, available for direct phone contact
- A personalised care plan with regular reviews to meet your care needs
- Access to a team of clinical nurses for guidance throughout your care journey
- Compliance checks for your care providers
- Budget management
- A flat fee of 15% of your Home Care Package, with no hidden fees or surcharges.
Self Managed PLUS
The Self Managed PLUS option provides support with care coordination. This service includes:
- A dedicated Care Partner, available for direct phone contact
- A personalised care plan with regular reviews to meet your care needs
- Access to a team of clinical nurses for guidance throughout your care journey
- Compliance checks for your care providers
- Budget management
- Care coordination
- A flat fee of 26% of your Home Care Package, with no hidden fees or surcharges.
Pricing
Both Self Managed and Self Managed PLUS options are charged at a flat percentage of your approved Home Care Package budget. There are no daily management fees, hidden costs, or surcharges. Our transparent pricing model ensures your Home Care Package budget is maximised for the care and support you need.
